Dear Doctor: My brother has learned that he has pellagra, Is it contagious?--R. E. H. Pellagra is a deficiency dis- ease, and is not contagious. It is a lack primarily, of a part of the Vitamin B group-- niacin, or nicotinic acid. How- ever, it is probable that there may be a lack of some of the other fractions of Vitamin B, as well as a shortage of some of the essential amino acids found in protein foods, particularly one called tryptophan. Proper diet and replacement of the missing nutritional ele- ments cure the disease, but if not properly treated, pellagra's symptoms are the Four D's-- diarrhea, dermatitis (skin dis- ease), dementia, and death. The dermatitis makes the skin as well as the tongue quite red, and blisters form. Digestive dis- turbances include diarrhea. The patient may become weak and anemia is common. The demen- tia is featured by - irritability, emotional upsets, depression, sometimes confusion, hallucina- tions and lethargy. RESPOND TO TREATMENT Treatment consists of a high- protein diet plus supplemental niacin. Fortunately, a patient responds rapidly to this treat- ment, although it may take many weeks before the skin re- turns to normal texture. Pellagra is distributed world wide but is seen more often in areas in which corn is an im- portant part of the diet, and the southern states are one such area. This does not mean that corn is bad in any reasonable quan- tity. It becomes so only when, over a considerable time, too much dependence is placed on it in the daily diet. Filling up on corn products at the expense of sufficient protein foods (meat, seafood, cheese, poultry, plus some protein in other foods) brings on the deficiency in niacin. It was noted by early investi- gation that corn might be in- volved, and subsequent study indicates that corn, besides lacking tryptophan, may also contain a substance which ac- tually is antagonistic to tryto- phan. There are 20 amino acids) which combine in various pat- terns to form the proteins which are essential to health. When one of them is lacking, trouble is bound to develop. In this in- stance, it is pellagra, Dear Dr. Molner: You write about hormones in menopause. A magazine recently had an ar- ticle about a medicine to 'make menopause obsolete."" But what about women like me who have cancer or have had it, and should not take hormones? No- body writes a word in our be- half.--Mrs, G, K. That isn't quite so. I have pointed out the use of mild seda- tives which can be very useful. So have other doctors. . Hor- mones are useful, in the ma- jority of cases but in my opin- ion it is going much beyond the facts to say that they make menopause "'obsolete." Dear Dr. Molner: My husband and I are both diabetic and therefore do not wish to have more children. Regardless of how the clubs are divided, you are now certain to make the contract. The purpose behind the fi- nesse is to guard against a pos- sible 4-1 or 5-0 division of clubs. If you neglected to finesse and attempted to run the clubs by cashing the A-K-Q, you might find yourself going down if you ran into an unlucky distribution of the adverse cards. The club finesse provides 160 per cent safety against every possible distribution. 2. The one thing to fear is that South will shiff to a low diamond after you've covered the jack of clubs with the queen and South has taken the ace. North may have the A-Q of diamonds and gobble up your K-J, or else you may guess wrong in choosing whether to play the king or jack on the diamond return. In either case, you would have to lose a spade trick eventually and go down one. However, there is a method of play that is practically sure to succeed. Duck the jack of clubs in dummy on the opening lead! (It is certainly reasonable to assume that South has the ace.) Regardless of what North plays next, draw trumps ending in dummy, and lead the king of clubs through South's ace. This permits you to discard a dia- mond on one of dummy's high clubs. All told, you lose a club, a diamond and a spade. (Yoy can ruff the fourth spade in dummy if the suit is not di- vided evenly.) If South sees through your scheme and elects to overtake the jack of clubs at trick one, it does not help him one bit. In that case, dummy's K-Q be- come tricks on which you can discard two spades. To Confer On Bill Of Rights OTTAWA (CP) -- Prime Min- ister Pearson said Wednesday he hopes he and the provincial premiers can agree here July 5 on guarantees of individ ual rights and freedoms to be adopted in provincial as well as federal law. Speaking in the Commons throne speech debate, Mr. Pear- son said he has invited the premiers to come here to be sworn in as members of the Canadian Privy Council in the presence of the Queen. Such a meeting would permit a free exchange of views on constitutional matters and how the country's political leaders see the future. Mr. Pearson said he hopes this exchange will produce in- dications of full agreement which could provide the founda- tions for later formal decisions. One thing the conference might start on would be the adoption by all levels of govern- ment of a bill of rights en- shrined in federal and provin- cial law. Before it could be hoped to make "structural changes" ih Canada's constitution, the fed- eral and provincial governments should be able to reach agree ment on a statement of common values. Such a statement would amount to a guarantee of the basic rights and freedoms of all Canadians, Mr. Pearson said he hopes this statement can be based on the Canadian Bill of Rights enacted by Parliament while Opposition leader Diefenbaker was prime minister and which would always be associated with Mr. Diefenbaker's name. Diabetic women can and do use the contraceptive pills. The pill may raise the blood sugar level somewhat, and in certain cases this may make the dia- betes less easy to control.
