Northern Affairs Identification of documents by JANE E. GREER There are many times in a person's life when he or she will be requir- - ed to prove his or her place and date of. birth, or will be asked for personal identification. For example, per- sons born in. Canada must now submit a copy of their birth certificate or certificate of Cana- dian citizenship in order to obtain a Canadian passport. Changes in~ regula- tions concerning Social Insurance Numbers also require applicants to support their applica- tions for Social Insuran- ce Numbers and amend- ments to Social Insuran- ce Numbers with copies of their birth certificate. Applications for Old Age Security and Canada Pension Benefits must also be supported by a birth certificate. Other instances where a birth certificate or identifying document is require include: an ap- plication for an Ontario Driver's Licence; en- rollment in Minor Hock- ey Associations; school enrollment; employment (some employers); bor- der crossings (into U.S.A.); premises licen- sed under the Liquor Control Act; and cashing cheques. In order to avoid embarassment or delays when asked for identi- fication our office sug- gests that everyone car- ry proper identification at all times. Firstly, we recommend that you ob- tain a copy of your birth certificate. This can be accomplished by making application to the res- pective Vital Statistics office in the province in which you were born. Our office has applica- tion forms for every province in Canada and in Ontario the fee is $5.00. We also recommend that Canadian citizens, either born in Canada or outside Canada, obtain a miniature certificate of Canadian citizenship. This document is plas- ticized and contains your photograph and other pertinent personal in- formation. The fee for this card is $8.00 and applications are avail- able at our office. For persons 18 years of age and over, the Ontario Government has made an Age of Majority card available and ap- plication forms can be obtained at-Liquor Con- trol Board outlets throughout Ontario and from Northern Affairs Offices. This plasticized wallet size card contains your photograph and date of birth. The fee for this card is $2.00. Finally, everyone should have a Social Insurance Number. Ap- plication forms are avail- able from U.I.C. offices and Northern Affairs Of- fices. There is no fee required for a social in- surance card, however, most people will have to produce their birth cer- tificate and what is known as a secondary document before the card is issued. For further informa- tion regarding identifi- cation documents or ap- plications contact the Ministry of Northern Af- fairs Office, located on the Lower Floor Penin- sula Building, 2 Gilbert Street, Marathon: tele- phone 229-1153 or ask the operator for Zenith 33160.
