Dear Dr, Molner; I had a cold and when nose drops didn't help my. doctor sent me to a nose specialist who said I had polyps. He .explained that it was a growth like a bunch of grapes. He want to operate, but I also read that you shouldn't operate on the nose, Can anything else be done? I am so afraid. He said it must be done while I am awake.--R.R. A good many year ago there was reluctance to operate on the nose because then we didn't know enough about how to do it. That time is long past. Polyps are small growths on a mucous membrane, usually. growing on a sort of stem, so clipping them off is quite simple with modern instruments. It can be done under a local anesthetic, You won't feel any pain, and it will be over quickly and you'll get up and go home. There is no other way of get- ting rid of polyps. ' Yes, it is possible to lose weight while at bed rest, but it means that the calories must be sufficiently restricted. You must also take care to get ample protein to repair your bone and tissues, This means lean meats, eggs boiled or poached (not fried, scrambled, or in omelets, since they add many non - protein calories); skim milk, cottage cheese, fish. You must keep fats at a min- imum because ounce for ounce fat far exceeds even sugar in calories. But you must also be sparing of carbohydrates--that is, starches and sugars. With such a restricted diet, you should have a multiple vit- amin supplement. Such 'a lim- ited diet should be prescribed by your physician and used with his approval. Gaining weight is not desir- able after a fusion operation on the spine, and you are wise to curtail any increase. I would suggest that, even while you are in bed, you keep yourself mentally active, read- ing, talking, doing handiwork-- anything to remain alert. There is a great deal of am- biguity attached to the term safety play. At times it refers ' to a play by which declarer 7 can guarantee no losers in a P given suit provided he handles a the combination of his own 4 cards and dummy's correctly. 1 Thus if declarer has A-Q-9-3- 4 and dummy has K-10-5-4, he i can protect against the loss of a trick in that suit by first a leading the ace. If it turns out 4 that either defender was dealt the J-8-7-6, declarer takes the indicated finesse against the jack. Note that if declarer's first lead is the king, he loses a trick if his left-hand opponent has the four missing cards. Another type of safety play is employed when declarer can afford to lose one trick in. a suit, but not two. Thus, with A-10-9-3-2 facing K-8-5-4, he may lead the two and, if the next player follows low, play the eight. This protects against the possibility of losing twa tricks if either defender was dealt Q-J-7-6. Still another form of safety play is directed not so much towards dealing with the com- bination of cards in a given suit as it is towards the ques- tion of dealing with that com- bination in a particular hand. Today's hand illustrates this type of problem. Declarer wins the queen of diamonds, cashes the king of hearts, and leads f another heart. When West fol- . lows low, South finesses the nine! Declarer does not expect the nine to win--he double-finesses because he guarantees the con- tract by doing so. If East wins with the ten or queen, South is sure to make four heart tricks and the contract. As it hap- pens, the safety play yields an unexpected overtrick. Without the safety play, South goes down. Ordinarily, declarer would fi- nesse the jack because that is the most lucrative play in the long run, but in tis deal he invokes the safety play because it assures three notrump.
