The aftermath of rheumatic fever often is far worse than the disease itself, but this de- pends on how the case is treated, and whether further attacks occur, St. Vitus dance occurs follow- ing some cases, but this usually subsides. The big danger is heart damage. This is the way the disease works: Tt ts the result of a strep in- fection, the same streptococcus germ which causes strep throat and sometimes other diseases. For some folks it is just a strep throat. But for others, the germ spreads through the body and becomes rheumatic fever. People aren't all the same. Some are subject to rheumatic fever; others escape it even though exposed to the same germ. The worst part of rheumatic fever is its attack on the heart. It may not seem dangerous at the time, but the heart valves, cularly the mitral valve, come inflamed. Then as the inflammation subsides, scar tis- sue is left, This can interfere with the free action of the valves. MAY RECUR People who have had rheu- matic fever may get it again if they encounter the germ, which is a common one. Each subsequent attack naturally means that damage to the heart valves will be intensified. Obviously the thing to do Is to avoid further attacks, This we are now able to do, by giving periodic doses of a long-lasting form of penicillin. The drug often is given at one-month in- tervals, and continued for a number of years. The result is that we are now preventing, or at least minimizing, heart dam- age which would have. been ex- tensive in years before we had this treatment. It is also important to treat a patient correctly at the time of the original attack--to subdue the infection as quickly as pos- sible, and protect the heart from excessive strain while it is in- flamed, The trouble used to be, and sometimes ¢till is, that the child's illness may not be recog- nized at first, or even at all. As to the young lady, Mr. B. obviously I cannot say whether she has remaining damage, or how much. A thorough physical examination, with emphasis on her heart, would give the an- swer. Since she knows she had rhev- matic fever, and was treated for it, we can assume that she had protective penicillin after- ward. Perhaps she atill does. And I trust we can assume that she knows that in her case a sore throat is a danger sign that should send her to the doc- tor without delay, Dear Dr. Molner: My doctor said that he found yeast cells in the vagina.
