However, the only pensioners who have been automatically enrolled in OMSIP are those who receive pensions under The Old Age Assistance Act from the Ontario Department of Welfare and those who have previously been receiving medical coverage from the Ontario 'Department of Welfare. These pensioners have already received their OMSIP registration cards. All other persons aged 69 and over who receive the Old Age Security Pension of $75 a month from the Department of National Health and Welfare, Ottawa, and are eligible for OMSIP coverage must send in an application form if they wish to join. Many of these persons will be entitled to full OMSIP coverage and benefits at no cost to themselves. Many others will qualify for partial premium assistance. Pensioners who have not already received their OMSIP registration cards must, there- fore, send in an application form if they wish to join the Plan. Application forms are available at all chartered banks in Ontario or may be obtained, along with any additional information required, from OMSIP, 135 St. Clair Avenue West, Toronto, Telephone 365-5911 or 365-5951.
