MOLNER, MD Dear Dr. Molner: Please ex- plain catarrh and what if any- thing can:be done about it I was told many -years ago by my grandmother that I had catarrh, At that time I had an unusual flow of mucus from my nose, I still have a drain- age into my throat, but not as bad as when I was younger.-- MRS. J. B. Catarrh is a perfectly good word which comes from the Greek word meaning 'to flow down," and is usually applied to a clear mucus - type dis- charge. To a mild degree this is normal -- our mucous mem- branes are intended to remain moist. When the flow increases heavily, we call it catarrh. Essentially this means some, sort of irritation is inciting the membranes to increase the flow as a means of combatting the problem. It is possible, with certain medications (of the "nose drop" type; among others) to reduce this flow or 'dry up the membranes." However, that doesn't get at lthe basic cause, which is the irritation: In fact, such arbi- trary drying up is actually an interference with what Nature is trying to do. Excessive use can damage the membranes, or lead to new irritation of a different type, or contribute to nosebleeds or such. Hence in general I oppose their use except very briefly. when a temporary drying up is worth the drawbacks. WHAT IRRITATES? Rather, the important point is to find out what is causing the excess flow -- what is the irritant? It can, of course, be a cold. Or chronic irritation from smoking or fumes. If the catarrh is seasonal, it strongly suggests allergy. Sometimes the cause is ana- tomical: Adenoids or _ tonsils; a deviated septum in the nose; polyps or some other obstruc- tion. Continued: catarrh is a nuis- ance but your chances of find- ing a cure on a do-it-yourself basis can be rather remote. Take this hand, where South is in three notrump and West leads a heart. Declarer plays low from dummy, East taking the king and returning a heart to the ace. South enters his hand with a club and finesses the jack on the return. East takes the queen and plays still another heart. Declarer wins with the queen and leads a spade to the king. Fortunately for South, it turns out that East has the ace of spades instead of West, and the outcome is that he makes three notrump. On the surface there appears to be nothing wrong with the play by either side. However, closer examination reveals that the contract should be defeated. East should resist the impulse to win the opening lead with the king of hearts, He should play low instead. If he does this, declarer then finds it impossible to make the contract. South would win the trick with the queen and prob- ably attack clubs, whereupon East would take the queen and return the king of hearts to the ace. As soon as East got in with the ace of spades, he would re- turn a heart to defeat the con- tract one trick. East is in a position to know, when oer, comes down, that South has all the missing high- card strength and that there is no hope of defeating the con- tract unless West has a long heart suit. East should therefore duck the opening lead, permitting South to win with the queen, because this play assures him of a later entry to West's hana so that the hearts can be run.
