It is the first of its kind in Durham Region. ~ Congestive heart fail- ure is the number one reason that people over 65 years of age are admit- ted to the hospital, and this number is increasing. The CHF Clinic, located in Room 115 Upper Ground at Lakeridge Health Oshawa, offers a multidis- ciplinary approach to treating and educating patients suffering from this cardiopulmonary dis- ease. "We are very pleased to open this service to the pp Ja | community today," said Brian Lemon, CEO, Lakeridge Health. "The CHF Clinic is another example of regional ser- vice that will benefit resi- dents across Durham and beyond. By cansolidating our services and offering many aspects of care in one clinic, patients have just one stop for their ser- WH THe, There's no better place to relax than a comfortable recliner or rocker and we'll help you find one that's just right for you! Under the direction of Shari Comerford, RN, CHF services Coordinator and Dr. Rakesh Bhargava, CHF Physician Coordinator, the outpatient clinic is dedi- cated to improving the lifestyle of these patients and their families by help- ing them to manage their fear and anxiety, educat- ing them about all aspects of their condition, encour- aging self-care manage- ment of heart failure, and improving access to urgent CHF care. The clin- ic team includes physi- cians, pharmacists, RNs, registered dietitians, physiotherapists and social workers, all working together to treat CHF patients holistically. In 1998, Lakeridge Health Oshawa, saw 288 admissions with a primary diagnosis of congestive heart failure. "Having this clinic up and running will have an impact on both patients who suffer from congestive heart failure and on our hospital," said Janice Dusek, COO, Lakeridge Health Oshawa/Whitby. "If patients are taught to be active participants in their own healthcare, they can learn to recognize early warning signs and how to treat them. This should reduce the number of emergency room visits and admissions." "The CHF Clinic is ded- "icated to improving the lifestyle of its patients," said Shari Comerford. "We can keep them out of crisis by immediate response to their clinical needs through appoint- ments or by phone, patient evaluations, edu- cation and regular moni- toring, and medical man- agement," said Comerford. "If you have experienced repeated admissions to the hospital or are experiencing a poor quality of life due to con- gestive heart failure, you should ask your doctor about attending our clin- ic."
