One of the most important aspects of ad- vance care planning is naming and having a conversation with a substitute decision mak- er - someone who will speak on your behaîf and make decisions for you - but only when you aren't able to do so yourself, said Baxter. Research has shown that advance care plan- ning significantly reduces stress, depression and anxiety in family members and caregiv- ers who know your wishes and can act with confidence on your behaîf. The information session wilI take people through the steps of advanced care planning and organizers will provide a workbook and other resources covering ail these steps.
