Nations World Health Organization IWHO hopes to declare the world free of small pox this year lf so it will be the first time disease which has scourged civilimtion for3000 years has been eradicated But how can the health agency be sure that the small pox virus is not still lurking in an animal in previously in fected areas or lie unidentified as yet in similar kind of dis ease ready to break out again The WHOS answer to these Questions is We are as sure as we can be The chief of the WHOS small pox eradication unit Dr lsao Arita and the units medical of ficer Joel Breman have been looking at the risks of the dis ease appearing again after its formal eradication Their conclusion is that it is unlikely The last known case except for two victims of laboratory virus in Birmingham England was in October 1977 and in volved 23yearold Somali who later recovered WAIT TWO YEARS The WHO has delayed pro claiming the world free of smallpox after $40million 12 year campaign until it has com pleted checks to make sure that two years pass without the reappearance of smallpox transmitted from one human being to another In an article for the latest WHO bulletin the first question the two smallpox specialists tackle is whether two years is long enough to be sure the virus is not lying dormant somewhere in the 77 countries of Africa Asia and South America where the disease was once endemic The WHO previously had set 18 months as the delay time be fore an official declaration that smallpox had been eradicated nationally but stretched this to at least 24 months because of varying surveillance capabi lities by housetohOUSe search teams The worlds last smallpox cases apart from the victims in Birmingham last fall were in remote Horn of Africa areas But Aria and Breman re ported The longest period that smallpox transmission contin ued undetected by national au thorities was eight months in Indonesia in 19711972 The twoyear period three times the longest delay in reporting appears to be adequate Of 181 countries only Cam bodia failed to give the Geneva based WHO information on lab oratories maintaining virus stocks In 1976 76 laboratories were reported to be holding the smallpox virus By the end of last year the total was down to nine and the WHO is aiming to reduce this figure to four by 1980 For now the world health body expects to retain stocks for comparison if smallpox type disease breaks out after eradication as well as keeping enough vaccine for 200 million to 300 million people in an emergency The WHO campaign was based on the belief that humans are the only host for the virus and smallpox could not be caught from animals There is no evidence to sug gest the presence of this virus in the natural environment EXAMINEI LOCALITIES During the last 12 months of the campaign before the last cases were detected on the In dian subcontinent and in Africa all localities with smallpox outbreaks were in vestigated the doctors said The infection was almost al ways shown to result from close llpox theiraim facetoface contact with other smallpox patients Only in one to five per cent of outbreaks was the source not identified But these outbreaks were near localities where other known cases had occurred The WHO dismissed fears that fragments of smallpox scabs remaining inside houses in previously endemic areas could cause new flareup of the disease It was reported in 1968 that virus remained viable in scabs kept at room temperature on shelf in European labora tory for over 13 years the spe cialists said But they added that more re cent experiments measuring virus decay under tropical con ditions indicated that the scabs were no longer infectious after three weeks AGREEMENTS SIGNED JAKARTA AFP ln donesia and the United States recently signed three agree ments for economic aid total ling $253 million to help finance several development projects The agreements will support rural work projects family planning and irrigation systems officials for both
