Haton Heathcare Services is now offerlng a Faits Intervention Cinic aimed at reducing the risk of fails for community-dweiling seniors in the area. The clinics are offered at both Milton District Hospital and Oakvile-Trafalgar Memoriai Hospital and are availabie f0 individuals 65 years of age and older, who have experienced one or more fals in the past year. Participants will receive a comprehensive assessment by a registered nurse, and a care plan recommending strategies thaf wilt reduce the risk of future fals. These may include referrals for physiotherapy, the use of walking aids, home safety measures or a plan f0 increase physical activi- ty. Clients will be followed up at three, six or twelve month intervals. This new service is available thanks f0 a four-year grant awarded f0 the Trauma Prevention Council of Central West Ontario by the Ontario Trilium Foundation.
