Connett argued that few studies have been done in relation to fluoride beyond its topical effectiveness for teeth. Connett also looked at numbers. He said 0.004 ppm (parts per million) is the level of fluoride in mother's breast milk. He noted 1.0 ppm is the level at which the public water supply is fluoridated. Connett also noted that 4.0 ppm is a level at which toxicity begins to be reached when it comes to fluoride evidencing itself in detrimental effects. The chemist, who just the week before had been in Israel speaking against the practice of fluoridation to university, municipal council and hospital officials, argued it's impossible to work safely within the one to four range. Connett opened his talk noting the many pro-fluoride arguments that have been made -- and many ludicrous accusations or descriptions that have been applied to him because of his stance. Connett argued the practice is continued based on past practice and that research and ongoing studies are not copious. The toxicologist said fluoride was never regulated as a drug in the U.S. because it was grandfathered, having been in use prior to 1938 when the Food and Drug Administration (FDA) was established. "Its main use before 1938 was as a rat poison," said Connett, noting, "It's bizarre." Noting what he dubbed a string of "bizarre" historical events, Connett noted that Brantford (with Sarnia as the control) was among a trio of test cases where artificial fluoride was added to the water supply in 1945. By 1950, he said, before any medical studies were done on anything but teeth, the U.S. Public Health Service endorsed the practice of fluoridating public water supplies. Though fluoride has been a much litigated air pollutant, Connett charged there was a move to detoxify the image of fluoride. Connett further charged that over time, the acceptance and endorsement of water fluoridation has been built mostly on past acceptance rather than review -- and that a hierarchy of endorsement can be traced to the sanctioning of the practice by government agencies. Connett claimed dental fluorosis is on the increase in the U.S. He also claimed no one was at last week's presentation to present the opposite viewpoint and that the media can be lazy by simply accepting the stance of established agencies instead of digging into the matter in detail. In Halton, the municipal water supply is fluoridated at 0.5 to 0.7 ppm levels, according to Halton Region's public health dentist Dr. Robert Hawkins. "The level has gone down in consideration of people getting fluoride from different sources like toothpaste," said Hawkins, explaining the 1.0 ppm level was used in the 1950s and 1960s when fewer people were using fluoridated toothpaste as part of their daily routine. "The important thing is the amount. Any substance in high enough concentrations or levels can be toxic," said Hawkins. Halton's chief dentist also noted the Canadian Dental Association (CDA) and American Dental Association (ADA) cautions people using local well water to have that water tested for fluoride levels as water can be naturally fluoridated in different geographic areas and could contain fluoride levels above those of the municipal supply. Health and dental authorities have no issue with community water that's fluoridated at appropriate levels, said Hawkins. In Halton, the municipal water supply is tested daily and Hawkins himself receives a monthly report as well as alerts if needed. Hawkins also said the CDA and ADA are aware of ongoing research into both the dental and other medical effects of fluoridated water and that the body of research to date gives rise to no negative health effects from consuming water fluoridated at optimal levels. Fluoridation at optimal levels is key, said Hawkins. On attaining high levels of fluoride from drinking a lot of water, Hawkins noted that if a person drinks copious amounts of water, they will also urinate often. "Even if you drink a whole lot of water it's impossible to overdose if water is fluoridated at the optimal level." The CDA, in its Position on Use of Fluorides in Caries Prevention, states, "The CDA supports fluoridation of municipal drinking water (at minimum levels required for efficacy as recommended by the Federal-Provincial Subcommittee on Drinking Water) as a safe, effective and economical means of preventing dental caries in all age groups. Fluoride levels in the water supplies should be monitored and adjusted to ensure consistency in concentrations and avoid fluctuations." The position paper continues, "Communities considering water fluoridation are encouraged to review their individual circumstances carefully and in detail, giving attention to any available data on the dental health of community members, the size of the group not likely exposed to adequate fluoride from other sources, the minimum level of fluoride required to be beneficial, and any other information, which would be helpful in making the required value judgment."
